How Boulder dropped no-shows from 40%+ to nearly zero

Yesterday, we launched the first CareOps event of 2025 with a standout session. Pia Shivdasani, VP of Operations at Boulder Care, walked us through how they broke away from traditional scheduling and created a pop-in clinic where patients can get care instantly—no appointment needed.

It was one of our best sessions yet.

  • No-shows are a system problem, not a patient problem. Traditional strategies like text reminders barely move the needle on reducing no-shows. Boulder Care recognized that rigid scheduling systems were the real issue, not patient behavior.
  • You can be scrappy and still make it work. The Pop-In Clinic didn’t start with fancy tech or a big budget. It began with one nurse practitioner, no new tools, and manual processes. It’s a reminder that you don’t need perfection to start–you just need to start.
  • Every new initiative needs a clinical champion—someone eager to jump in, test the waters, and make it work. Having a clinician like Jodi, who embraced the experiment and brought energy to the project, was key to the success.
  • When you see positive signals, don’t wait for perfect data—it doesn’t exist. Take the leap, even if some KPIs dip during the transition. Stick to the plan, execute with focus, and trust that the metrics will stabilize and improve as the model takes shape.
  • Early on, Boulder team made the Pop-In Clinic feel like more than just another task. They created a sense of excitement and camaraderie, turning it into something the team wanted to be part of. Adding fun is an underrated but powerful way to motivate teams and keep the momentum going.

Kudos to Boulder Care for their leadership and the incredible results they achieved. Their work shows what’s possible when you rethink the status quo and put patients first.
